Procedure:

The referral process can be initiated by phone, fax, mail, or e-mail.  An admissions worker will contact you within 48 hours of receiving the referral.  Pre-placement interviews will be conducted at the location of your choice..

Referral Material Should Include:

• Cover letter indicating reason for referral
• Psychosocial Assessment of a child and family including all prior services received and custody status
• Current psychosexual evaluation or risk assessment
• Current neuropsychological evaluation
• Current psychological/psychiatric evaluations
• Police Reports and Court records
• Victim Statements
• Medical and Dental records
• Immunization Records
• Current Individual Education Plan
• Complete School Records
